About the Timiskaming Ontario Health Team Diabetes Initiative
This Diabetes Initiative aims to deepen an understanding of the current state of local diabetes care to inform sustainable, quality improvement actions. This work is guided by the Quintuple Aim, focusing on improving patient and provider experience, advancing health equity, improving population health, and improving value. Through a learning system approach this initiative will ensure the essential input of providers and people living with diabetes.

The Timiskaming Ontario Health Team is one of 58 OHTs in Ontario serving the Timiskaming District and Temagami. Ontario Health Teams are providing a new way of organizing and delivering care that is more connected to patients, families and care partners in their local communities. Health care providers work as one coordinated team – no matter where they provide care, to strengthen local services, make it easier for patients to navigate the system, and create seamless transitions between providers.
